I, Sunil Thomas, son of Thomas Kurian, residing at Flat 5B, Lakeview Apartments, Kakkanad, Kochi 682030, holder of passport no. N7654321, am the father of Master Aaron Thomas, born on 14 February 2018.
I have no objection to a passport being issued to my son Master Aaron Thomas. The application is being submitted by my wife, Mrs. Anju Thomas, and I consent to her signing and submitting it on behalf of the minor.
I confirm that there is no court order restraining the issue of a passport to the minor and that the minor is not the subject of any custody dispute.
Signed
Sunil Thomas, Father

Stamp paper, notary and signing
Annexure D is a plain declaration, no stamp paper. If the parent is abroad, it should be attested by the Indian Embassy or notarised locally. Separated parents without the other's consent use Annexure C instead.
