Affidavit for date of birth

If you were born before birth registration was common, or your birth was never registered, a date-of-birth affidavit sworn by you (or by a parent for a child) is accepted by passport offices, schools, EPFO and courts along with a secondary proof like the SSLC certificate or a hospital record.

What it must include

  • Date and place of birth, written in words as well as figures if possible
  • Why no birth certificate exists (not registered / records lost)
  • The secondary document that shows the same date (SSLC, school leaving, hospital record)
  • The office it is for
  • Verification, signature, notary attestation

Common mistakes

  • Date in the affidavit not matching the SSLC certificate.
  • Omitting the place of birth.
  • For a child, the affidavit must be by a parent, not the child.

Stamp paper, notary and signing

Non-judicial stamp paper of Rs. 10–100 depending on the state (Rs. 10 in Kerala). Notarised. Passport Seva accepts it along with a Non-Availability of Birth Certificate (NABC) from the municipality for people born after 1989.

Common questions

Is this enough for a passport?

Born before 26 January 1989: the affidavit plus SSLC/school leaving certificate is usually accepted. Born after that: Passport Seva expects a birth certificate; get a late registration or NABC and attach this affidavit.

Can I use it to correct a wrong date of birth?

No. That needs a correction in the primary record (school/municipality) with supporting evidence, sometimes a court order.

Does EPFO accept it?

EPFO accepts a notarised DOB affidavit only together with another listed proof such as the school certificate.
