Colleges (especially for MBBS/NEET counselling, engineering and law admissions) and some employers ask for a sworn explanation of any gap of a year or more between two courses or jobs, confirming you were not enrolled or employed elsewhere and not involved in anything unlawful.
I, Nithya Balakrishnan, aged 20 years, daughter of Balakrishnan Nair, residing at Nithya Nivas, Pala, Kottayam, Kerala 686575, do hereby solemnly affirm and declare as under:
1. I passed the Higher Secondary (Class XII) examination of the Kerala Board in March 2024.
2. I did not pursue any course or take admission in any institution during the period from April 2024 to September 2026.
3. The reason for the said gap is that I was preparing for the NEET-UG examination and attending to family responsibilities after my father's illness.
4. During the said period I was not employed anywhere and was not involved in any activity contrary to law.
5. This affidavit is made for submission to the Kerala Medical Counselling authority for admission to MBBS 2026-27.

Names, numbers and dates above are examples. Replace them with yours.
Rs. 10–100 non-judicial stamp paper by state (Rs. 10 in Kerala, Rs. 100 in Maharashtra). Notarised. Some colleges want it in their own "gap certificate" format — use theirs if provided.
Not by itself for most courses; the affidavit simply records it. Some institutes cap gaps at 2–3 years.
