Affidavit for name mismatch (one and the same person)

Your SSLC says "Anjali R", Aadhaar says "Anjali Rajan" and the passport says "Rajan Anjali". Banks, EPFO, passport offices, universities and insurers ask for a "one and the same person" affidavit before they accept documents with different versions of your name. It is not a name change; it says the variations are all you.

I, Anjali Rajan, aged 32 years, daughter of Rajan Nair, residing at 12 Rose Villa, Kakkanad, Kochi, Kerala 682030, do hereby solemnly affirm and declare as under:

1. My name is recorded as ANJALI R in my SSLC certificate (register no. 123456, March 2010) and in my degree certificate.

2. My name is recorded as ANJALI RAJAN in my Aadhaar (XXXX XXXX 4321) and PAN (ABCPA1234F).

3. My name is recorded as RAJAN ANJALI in my passport no. P1234567, where my father's name appears as the surname.

4. ANJALI R, ANJALI RAJAN and RAJAN ANJALI are all the names of one and the same person, that is, myself. The differences arose only from the way initials and surname were written in different offices; I have not changed my name.

5. This affidavit is made for submission to the Employees' Provident Fund Organisation for correction of my name in the UAN records.

Verification: Verified at Kochi on 2 November 2026 that the contents are true and correct.

What it must include

  • Every version of the name in capitals, each with the document and number it appears in
  • The "one and the same person" line
  • Why the difference arose (initials, surname order, spelling) — one line
  • Which office it is for
  • Verification, signature, notary attestation

Common mistakes

  • Listing only two names when a third document has yet another version — list all.
  • Calling it a name change; that is a different affidavit and triggers Gazette requirements.
  • Document numbers missing, so the officer cannot match them.

Stamp paper, notary and signing

Non-judicial stamp paper of Rs. 10–100 depending on the state (Rs. 10 Kerala, Rs. 20 Karnataka, Rs. 100 Maharashtra/Delhi), notarised. EPFO, banks and insurers accept it as-is; the passport office may also want a newspaper advertisement if the names are very different.

Common questions

Is this enough for EPFO name correction?

EPFO's joint declaration form plus this affidavit and Aadhaar usually works for minor differences; major differences need the employer's endorsement.

Passport shows surname first — do I need to fix it?

Not necessarily; this affidavit explains it. For a permanent fix, apply for a passport re-issue with the corrected name order.

Can my spouse's name difference be covered in the same affidavit?

No; each person swears their own.
