Affidavit for a lost or damaged passport

To get a re-issue after losing your passport in India, Passport Seva asks for a police report and an affidavit in its Annexure F format stating how and where the passport was lost, whether you travelled on it, and that you have not misused it. If lost abroad, the Indian Mission issues an Emergency Certificate or a new passport with a similar affidavit.

[Plain-English version of what Annexure F asks; use the official form for the application]

I, Rahul Varma, son of Mohan Varma, residing at 7 Marine Drive, Kochi, Kerala 682031, do hereby solemnly affirm and declare as under:

1. I was issued Indian passport no. P1234567 on 10 May 2019 by the Regional Passport Office, Kochi, valid until 9 May 2029.

2. The said passport was lost on or around 20 October 2026 while travelling by train from Kochi to Bengaluru. I noticed the loss on reaching Bengaluru and searched thoroughly without success.

3. I lodged a complaint with the Kochi City Railway Police on 22 October 2026 (complaint / FIR no. 456/2026), a copy of which is attached.

4. I last travelled abroad on the said passport to Dubai from 2 to 9 March 2026. I have not travelled on any other passport and I have not been deported or refused entry by any country.

5. I have not misused the passport, nor has it been impounded or revoked. If the passport is found, I undertake to surrender it to the nearest Passport Office or Indian Mission.

Verification: Verified at Kochi on 2 November 2026 that the above contents are true and correct.

What it must include

  • Passport number, date and place of issue, validity
  • How, where and roughly when it was lost; what search was made
  • Police complaint / FIR number with a copy attached
  • Travel history on the lost passport (last trip), and that no other passport was used
  • Undertaking to surrender if found
  • Verification, signature, attestation (Annexure F requires a notary or magistrate)

Common mistakes

  • Guessing the passport number — get it from an old photocopy, visa stamp or the airline's record.
  • No police report; Passport Seva will not process without it.
  • Hiding a previous travel or deportation; it is on record and leads to rejection.

Stamp paper, notary and signing

Annexure F is executed on plain paper and attested by a notary, magistrate or oath commissioner; no stamp paper is required by Passport Seva, though notaries in some states insist on a Rs. 10–20 stamp. Submit online via passportindia.gov.in (Re-issue → Lost/Damaged) with the police report.

Common questions

Do I need the official Annexure F or will this do?

Use the official Annexure F wording from passportindia.gov.in for the application; this page explains what it says. Our generator makes the general lost-document affidavit, which is useful for other offices (bank, airline refund).

Lost abroad — what first?

Report to local police, then contact the Indian Embassy for an Emergency Certificate or re-issue; take the police report and any passport copy.

Damaged passport — same process?

Yes, Annexure F covers damaged passports too; no police report is needed if you still have the damaged booklet.
